The tortuous Cold of Jammu City 2023

Since the end two week Calender of the last month of Dec 2022 . New Year 2023 has stepped towards harsh cold and has hitted Jammu city with intense cold. There are two scenarios which are on ground in view of people who used to migrate from other colder regions like parts of Srinagar, Doda, Kashtiwar & Pir Panjal towards Jammu City. But the over view is totally different regarding the first one. Secondly the years of trend has been turned to a less mobilization of people to Jammu city in view of the Darbar move. Since the UT has came into force Secretarial work is being done according to will of people at both places which have given people less chances to move towards hotter region. Apart from all these conditions if anybody want to move he prefers not to move because the intensity of cold changes at the peak in Jammu City. An emerging and health deteriorating situation presently in India part of Kashmir is of appearance in silent diseases, Non-communicable diseases and End stage diseases. All these types of diseases hit the population folks with increasing life expectancy and risk factors of change in lifestyles.  In the modern age life expectancy is increasing and deaths are either due to Cardiovascular diseases which play an important role in silent and non - communicable diseases. A pilgrimage to Holy Cave and Medical Services on board.

The Amarnath Yatra called as SANJY 2022 (Shri Amarnath Ji Yatra) is a yearly pilgrimage of Hindus on distant Peaks of Pahalgam towards Chandanwari, Sheeshnag and Panjtarni as an ancient route discovered. Across all parts of india the piligrims take part as devotees to visit Holy cave which has an accumulation of Ice piller called as Shiv Linhgam. Yatra is of approximately 45 yrs duration. Non Communicable disease viz Hypertension, Diabetes Millatus, Cardio vascular disease, Obesity, Cervical Cancer, and Oral Cancer pose a great threat to the physique and immunity once the person with above co-morbiditiy is Contracted with the Corona virus which is Contiguous and 4 times Virulent in the present than previous year.  During the present pandemic greatest tool is to keep masked up with a type of N95 and atleast 20 secs handwash after touching any sort of object or material. Diabetes Millatus : A hidden disease in masses & its Prognosis.

Diabetes Millatus : A Non Communicable disease which is a threat to masses of inhabitants of Poonch J&K UT.  Incidental finding ratio is 3-4 patients out of 10 Patients. The patient age group fall from yongester's to old age, both men and women. Evaluating a patient on pulmonary issue , Cardiac ailments, Neuropathy disorders, Urinary tract complaints and on Gastrointestinal symptoms cum signs is a key to reach to it's incidence among masses.  While proper scanning of patients with immediate Random Sugar checkings and with HBA1C, a marker to see to what extent the Diabetes has reached in their body.  A spike of 3 times is usually seen. With the start of the day usually in Govt health sector OPD protocols for ILI and ARI even Common flu's has given a tough fight to distinguish and label it as Corona. Usually testifying the patient in separate Flu OPD's is a great value triage for possible watch out on the present spread. Be In Govt or in day off Privatization. A medic is well versed to treat patients with utmost care.  Pandemic has given a clear message to epidemiologists and to Public Health about it's way of appearance, change in it's spread, Time of spread, Climatic re-spread, mutation, morality, and existence of new research has pushed it's discovery for a New Vaccine.The Vaccination to control it's roots on right time is the only way out. 

RT-PCR /RAT prior to a Covid 19 Vaccine Jab : A Healthy Initiative.

RT-PCR testing or RAT is mandatory in active phase of infection. The Vaccination is basically promoted in a suppression period but the Covid19 has left no way to drove population's towards mortality.  Before taking Jabs a Sensitive or a Specific test must be done simultaneously. If RAT is negative go for RT PCR. If you feel all ok get the jab.  If positivity comes in person during testing. He should refrain from taking Jab. Once he is all ok after isolation and treatment which takes weeks to withstand again in recovery and fight against infection then a person is free to take the jab. But all precautions, masking  & social distancing should be on accountability as a fashion in letter & spirt ! 

COVID19 Vaccination : Beyond Million's

#Lancet says :  Simple mathematics helps. If we  vaccinated a population of 100 000  and protected 95% of them, that would  leave 5000 individuals diseased over  3 months, Rather, a 95% vaccine efficacy means that instead of 1000 COVID-19 cases in a population of 100 000 Without Vaccine (from the placebo arm of the above mentioned trials, approximately 1% would be ill with COVID-19 and 99% would not) we would expect 50 cases (99·95% of the population is disease-free, atleast for 3 months)  So this is only to save from Third Wave. 

Policy of Vaccination : A tool to Counter COVID-19 in J&K UT

The role of Vaccination in Jammu & Kashmir State came in force in early November 2020  when the Govt has introduced first time the vaccine COVISHIELD against Covid 19 in Jammu and Kashmir State. The Vaccine was been given to population category wise . First category sorted was Frontline workers, PRI's. This runned till few months and later on the guidelines came in February 2021 for the second category of people age of above 45 with comorbidities and later on was introduced for all population above 45 yrs ago. In this particular division of Jammu, Distt poonch got 86% of Vaccinated people with ist dose. The second dose is in force till it's clearance & dose administration to population till 20th June, 2021.  On 21 June 2021 as per guidelines by State Govt all population from 18-44 will get their jabs in their concerned Health care stations too. Covid19: Has multiple Strains & likely shows Negative Lab results after repeating it in Administrative Quarantine Centers on 2nd & 3rd time.

It's a big Yes to Covid19 for saying bye bye as resistance is developing in few of the persons recently detected with Positives on arrival from other states. On resampling same persons after few days repeatidly got negative results but remained asymptomatic and no signs on chest xrays.  Finally despatched to their homes after completion of admin qurantine. 9 persons were discharged to their back home through a legal health protocol. Warm wishes for them on departuring via Ambulances from our Block. Moreover we are hoping for a Vaccine to come after resistance develops in a phased manner in community. This vaccine can bring miracles But it should come for all strains.  But following of sanitization and masks can stop it in community contamination. Hygiene maintainance and social distancing is also a tool to avoid it's spread.  Hence, watching the scenario in block precisely. May it end soon.

Block Level administration should take a note about the fact for proper care and induce a practice to understand the safety of health workers and the frontline medicos who come close in contact with the positive cases in a limited and less protective gear. The state provided a good set up and budget to deal with this state level epidemic and a pandemic announced by WHO few months back. Now the situation in our District Poonch was quite normal one weak before. But unfortunately we fall in prey due to travellers comming from all other Indian states who were not testified there and were sent like folks and dispatched in buses. The cases are maximum from the outside, citizens of J&K doing their different level of businesses outside on routine.  The process is going on in a smooth manner to sort out the travellers on frisking points and sent to block level qurantine centers.
